How to submit your COVID-19 status

Everyone is required to either submit proof of COVID-19 immunization or request an exemption

All students, faculty and staff should upload your proof of vaccination (including booster) online. You will be required to complete a short form with the type and dates of your vaccine and upload your photo or image. Previous vaccine card images are stored in the system but will not appear.

All University members should refrain from asking others about their vaccination status as such dialogue could prompt disclosure of confidential medical information, which is protected under the ADA (Americans with Disabilities Act of 1990).
