Due to Illness or Injury

When a student is absent from class due to illness or injury, it is the responsibility of the student to communicate with his/her professor and to follow the requirements of the professor regarding the course work missed. Penalties for absenteeism depend upon the policy and discretion of the professor, as outlined in the course syllabus.

Written notification to professors regarding student absenteeism due to illness is provided by the Director of Health Services only when a student is hospitalized or has an extended illness requiring three or more consecutive days of absence or hospitalization.

Professors wishing to verify cases of student absence due to illness or injury may call the Director of Health Services at 814-362-7631. Health information is confidential. Therefore, a Release of Information must be signed by the student prior to disclosing any health information, including date(s) of contact.


We strongly urge each student to meet these requirements before arriving to campus. Making arrangements to meet these requirements once you are on campus could be costly, time consuming and involve off-campus appointments. All first time, full-time and part-time students must provide proof of the following:

  • MMR VACCINE (Measles, Mumps and Rubella)
    Two doses of MMR (Measles, Mumps, Rubella) vaccination at least 28 days apart after 12 months of age
    Two doses of varicella-containing vaccine at least 12 weeks apart if vaccinated between 1 and 12 years of age and at least 4 weeks apart if vaccinated at age 13 year or older.


A history of immunity to these diseases proven by serological evidence showing adequate antibodies satisfies this requirement. Talk to your medical provider about having bloodwork drawn (a “titer”) to prove immunity.

    One dose of meningococcal conjugate vaccine (MCV4) administered at age 16 or older
    OR Sign a waiver declining the vaccine. The waiver form is available on the Health Services page on the University’s website.

Places you can contact to obtain a copy of your immunization record:

  • Your doctor or pediatrician
  • Your state health department
  • Schools you attended in the past

New incoming full-time students who fail to provide written proof of required immunizations will be prohibited from registering for any classes beyond the term of first enrollment, on any campus of the University of Pittsburgh.

Students entering the nursing program have additional requirements unique to that program. Those requirements will be communicated to you by the Department of Nursing.

The University of Pittsburgh at Bradford Student Health Service is required by law to maintain the privacy of your medical records and to give you this Notice that describes our privacy practices. This Notice describes how we may use and disclose your protected health information to carry out treatment, payment, and health care operations, and for other purposes permitted or required by law. It also describes your rights to access and control your protected health information, which is information about you, including demographics that may identify you and that relate to your past, present, or future physical or mental health, and related health care services. This Notice takes effect September 16, 2016 and will remain in effect until we replace it.

We are required to abide by the terms of this Notice of Privacy Practices. We reserve the right to change our privacy practices at any time. We reserve the right to make the changes to our privacy practices and this Notice, effective for all health information that we maintain, including health information we created or received before we made the changes. Before we make a significant change to our privacy practices, we will change this Notice and make the new Notice available. One of our functions is to manage patient medical records during a student's tenure at the University. Each record is necessary to provide quality healthcare to each patient. Since medical records contain personal information, we adhere to all laws and guidelines which govern the use and disclosure of such information.

A patient may receive a hard copy of his/her medical records or may have a copy forwarded to his or her personal healthcare provider by completing the necessary authorization form (available at Student Health Service). A nominal fee may be charged.
